About UsThe mission of the Como Park/Falcon Heights Living at Home Block Nurse Program is to organize community volunteers, professionals, and family members to provide care for their neighborhood seniors in ways that support choice, dignity, and independence.
The Como Park/Falcon Heights Living at Home Block Nurse Program was founded in May of 1993 by District 10 resident Audrey Roedl. Since then, the program has continued to grow. We are supported by private donations, a MN Live Well at Home grant, and a combination of city, county, and state funding. We provide services to residents aged 65 and better in the community of Como Park as well as areas of Falcon Heights. With the spirit of “Neighbors Helping Neighbors,” we provide a spectrum of services, bringing neighborhood health professionals, volunteers, caregivers, and community leaders together to provide high-quality, long-term care.
