Live Well at Home Screenings
Staff will provide a screening to discuss your health concerns and goals and help you develop an individualized care plan.

Volunteer Support
Volunteers provide valuable services such as transportation to and from appointments and grocery stores, chore support, and most importantly, friendship.

Homemaker
Our Homemaker is able to provide light housekeeping duties.

In-Home Foot Care
Foot care is coordinated by our Nail Technician.

Community Outreach
Community outreach includes education and blood pressure clinics.

Caregiver Support
Two monthly caregiver support groups are hosted by the Living at Home Block Nurse Programs, one by St. Anthony Park Area Seniors and the other by Hamline Midway Elders. All are welcome.

Service Coordination
We are able to meet with seniors and families to discuss their needs. A care plan is designed for each senior that may include a variety of volunteer support or professional referral services. 

Service Area
We serve the Como Park and Falcon Heights area. Our boundaries are roughly Roselawn/Snelling/Railroad Tracks on the west half and Larpenter/Dale/Maryland on the east half.   [Map of Service Area]
